How do I schedule an appointment?

Are there patients or conditions that you are unable to treat? 

At this time, we are unable to accommodate patients in the following situations:

  • Court ordered care

  • Children in care of Arizona Department of Child Services

  • Co-Occurring Disorders (Addiction or substance abuse with a mental health disorder)

  • Children < 12 years old

  • Individuals on Long-Term Disability for a Mental Health Disorder

  • Individuals diagnosed as Seriously Mentally Ill (this may include Schizophrenia, Schizoaffective Disorder, Schizotypal Disorder,

  • Patients on Benzodiazepines (Xanax/Alprazolam, Ativan/Lorazepam, Klonapin/Clonazepam, Valium/Diazepam, etc). 

This practice does not provide ANY prescriptions for benzodiazepines and we are unable to manage patients on these medications. 

***Please note that if you have an active prescription for benzodiazepines, you will be provided with a referral to a qualified psychiatric care provider, outside of Bella Vita Integrated Healing, who is able to manage your care. We apologize for any inconvenience this may cause. 

Do you accept insurance?

Yes, we currently accept many insurance plans at this time. We are NOT in network with any plans other than what is listed. We do NOT accept any Medicare or AHCCCS plans. We pride ourselves on the amount of time we can dedicate to our patients. If we are not in-network with your insurance, you will be responsible for any fees not covered by your insurance (including labs, imaging, testing, and medications). If desired, we are happy to assist you in providing an invoice and/or superbill for your to submit to your insurance for reimbursement. Please note that reimbursement is not a guarantee. You should always check with your insurance carrier before receiving any care. 

When is payment due?

All self-pay, copays, co-insurance, and deductibles, are expected to be paid at the time of service. Failure to pay, may affect your ability to schedule future appointments or result in discharge from the practice. We are a small practice that is dedicated to your health and well-being. Please help us by paying your bills on time.

What forms of payment do you accept?

Visa, Master Card, American Express, Discover, HSA, FSA

Do you offer a payment plan or care credit? 

Do you charge a no-show or  late cancellation fee ?

Yes. We have reserved this time especially for you, please help us by keeping your appointment. We do understand that life happens and plans change, illness occur, please give us at least 48 business hours notice. Appointments cancelled less than 48 (business) hours prior to the time of service, or no-shows, will be charged the full self-pay rate for the appointment that is booked.
